At the behest of OME NY created two statewide Student Record forms to insure consistency of data collection across the MEOPs.
Summer Student Record Form
The Summer Student Record Form should be printed out from MIS 2000 at the beginning of the summer session. The forms printed in this way will have some of the information already filled out.
The forms are then distributed to the tutors to collect additional information.
The forms are returned to the MEOP's office so that the data can be put on MIS 2000.
School Year Student Record Form
The School Year Student Record Form should be printed out from MIS 2000 at the beginning of the school year. The forms printed in this way will have some of the information already filled out.
